Completed PD Credit Request (Click here once you have already attended a PD)

Once you have completed any professional development session (in-district, out-of-district, online sessions), you must complete the Completed PD Credit Request form below to receive credit AND you must send documentation that you attended the event to Janice Marcum via courier in order to be awarded PD credit.  
At the end of each 9 weeks, your Principal will receive an email with all your professional development credit hours.  However, it is your responsibilty to keep track of all PD sessions you attend and copies of documentation as well.
